Personal Training Liability Waiver and Agreement

1. Assumption of Risk
I understand that participation in physical exercise, training activities, and fitness programs involves inherent risks, including but not limited to, muscle soreness, sprains, broken bones, heart attacks, and other serious injuries or health issues. I voluntarily assume all risks associated with participation, whether known or unknown.

2. Release of Liability
In consideration of being allowed to participate in training sessions provided by BodybyBamford, I hereby release, discharge, and hold harmless BodybyBamford, its employees, agents, and representatives, from any and all claims, demands, rights, and causes of action of any kind arising out of or connected with participation in fitness training.

3. Medical Clearance
I affirm that I am in good health and capable of participating in exercise activities. I have either had a physical examination and have been given my physician’s permission to participate or I have chosen to participate without the approval of my physician and accept all responsibility for my participation.

4. Scheduling and Cancellation Policy
Cancellation Policy: At least 24 hours’ notice is required to cancel or reschedule a session. Failure to do so will result in a forfeited session without refund.

Trainer Cancellation: If BodybyBamford cancels a session, the session will be rescheduled promptly without additional charge.

5. Payment Terms
Payment must be made by the day of each training session. Accepted payment methods: Venmo, check, or cash. No refunds will be provided for unused sessions unless otherwise agreed in writing.

6. Confidentiality
All personal information, fitness assessments, and client progress records will be kept confidential and will not be shared without client consent.
